We have secured a block of rooms at Dreams Onyx with exclusive rates for this group. It is especially important that all guests book their accommodations within this wedding reservation that Kelley and Richard have arranged through Forever Us Destination Weddings & Honeymoons. This will ensure that all guests are recognized as part of this wedding group, guaranteed a room at the wedding resort, and you are included in the wedding events and activities without incurring any added costs. We encourage you to act early to assist the couple in being able to start their wedding planning with knowledge of how many guests will be in attendance. Payment plans are available for room reservations.

*****IMPORTANT TRAVEL REQUIREMENTS*****
- Travel to Dominican Republic requires a valid passport book for all travelers.
- Your passport book is required to be valid for 6 months after your RETURN date to be given entry to Domincan Republic, it must be in excellent condition and have at least two blank pages available.
- Names on all reservations must match the name as it appears on your passport book.
- All travelers are required to be booked in advance. Any traveler whose name does not appear on a reservation will be denied resort access.
- When traveling with a child without both custodial parents, a notarized letter from the absent parent should accompany you in travel for TSA purposes. Contact TSA for more specific details.
DEPOSIT:
Deposit amount of $200 per room is due to confirm your reservation. The cost of travel insurance, if elected, is additional. The cost of insurance is based upon the cost of your travel arrangements. The policy must be purchased at the time of deposit and cannot be added after your booking is completed. Details will be provided by your travel advisor.
